Fortunately I dont have back or neck problems where I would need a special strap but I imagine this one could help.This is a great saxophone neck strap. The padding makes it comfortable and it has served me well for 6 years. Possibly the best feature is the hight adjustment buckle which allows instant adjustment to allow for clothing, different sized saxes etc.without slipping. I have other straps but this is the one I prefer to use. Sorry, I can't think of any cons.

This strap has made life much easier on a day to day basis. I suffer from back and neck pain, as many of us do, and the design of this strap really does spread the load. The padding ensures no direct wieght on the back of your neck

Very comfortable. Usually I have problems with numbness in my fingers and hands after playing tenor and bari for a while, but not with this strap.

Very comfortable strap. I feel the large is a little bit on the larger side for Alto sax, considering I am not a small person, but it still fits and I haven't tightened it all the way through. You can also easily shorten it by tying a couple of knots.
